There are multiple ways to join the CP Register.

It is possible to join the CP Register online or if you prefer you can download and complete both the registration form (PDF) and consent form (PDF) and mail these to your local CP Register.

Alternatively you can give permission to your health practitioner or education professional to submit your details on the CP Register website or send your name and contact details to your local CP Register and we will contact you to complete the registration.

Details will be verified when the CP Register receives a registration. Consent must be provided by the person with cerebral palsy to use this information. All reported information will be de-identified for public reports to protect the person with cerebral palsy’s identity. This can be done online via the website, or manually by downloading the form and send it back to us.


Each person must complete a consent form to participate – Download the consent form
If you are going to enter or provide details of a Cerebral Palsy Record, we must obtain consent from the individual with Cerebral Palsy to ensure that he/she is consented that their information will be used for research purposes and could be seen by the person who register the record. The form must be completed and signed by the individual with Cerebral Palsy and send back to us to complete the registration.

Consent Removal Form – Health Professional – Download the form
To withdraw consent for a nominated health professional to be contacted to verify or provide information on a cerebral palsy record, the attached form must be completed and returned to the Cerebral Palsy Register.

To withdraw consent to receive invitations to participate in research projects, the attached form must be completed and returned to the Cerebral Palsy Register.
